How do you open it to fill it with water? I can't seem to get it open.

Same problem. Answer turned out to be “yes, turn it so hard you think you’re gonna break it.” Whoever packs those sealed it tight.

Why didn't this bladder come with the back pack holder as is described in the photos? That's the reason I bought this product

Mine had one, it broke, but it has a hook on the opposite side of the handle.

Is this bladder certified food-grade, bpa-, bps-, phthalate-free?

According to the vendor's description it is BPA free. There was no mention of BPS or phthalate that I could find. It is made of TPU material, so perhaps a search for the properties of the material will provide more information.
