Do they have a Heam on the bottom for small weights?

They do have a small heam at the bottom. They do need some weights in them somewhere. Mine blow around a lot during the day when trying to block sun from porch.

do they come with the tie back

There are no ties that come with it.

Does the measurement of the curtain take in consideration the pleats?

The curtains are a true measurement size if you lay them flat, they are the exact measurement that the package states.

does the 144 in curtain rod need a support in the middle?

I would definitely use a support. The curtains are of a heavier material.
