how is it attatched to car seat?

It doesn't attach to a car seat. It is intended to be played with on the floor or other flat surface.

Is it possible to install in the stroller?

No it does not have a way to install it to a stroller.

Has anyone use it in the car while child was in car seat? Looking for something to mimic the car so my son can "drive" like us.

Granddaughter loves to drive in the car with the VTech Turn and Learn Driver. Loves the sounds.

I got this for car rides for my toddler to be busy will be drive places. He loves it, he normally throws things on the floor when he's done with him and they don't last long but this one always stands a test of time and has it for ages
