I just built a new PC with this four days ago. The fan that comes with the Heat Sink is fairly loud when it starts to ramp up... however, DON'T LET THAT BE WHAT STOPS YOU FROM BUYING THIS! If you find that the included fan is too loud for your tastes, you can EASILy replace it: the heatsink fan is just a standard 120mm fan that's attached to two plastic adapter/clips that clip onto the sides of the heatsink. It also comes with a second set of these adapters so you can have a fan pushing air into the heatsink on the front and a second fan pulling the air out through the back. So if you want something more silent, you could buy two quieter 120mm case fans and have one on each side of the heatsink to ensure that enough air is still getting through without the added noise.

Should the fan be facing the blower that exits the back of my case?

You should choose the path of air going through your case and point all the fans in the same direction. So if the fan in the back of the case is drawing from the case and blowing to the outside, the Cooler Master fan should blow toward the rear fan and allow it to exhaust the air to the outside.
