I noticed that the glass breaker pin is not exactly aligned with the opening (aperture). Did I get a defective piece or is it by design?

I believe the end contains a steel ball. I would try it on a piece of glass to see if it breaks. Of course use safety glasses or gloves. The other suggestion would be to email the company.

Does the TSA allow this on planes? Anything with a blade tends to be disallowed/confiscated

The short answer is call TSA and ask them however if you put it in your check in luggage it should be no problem.If you try to carry it on in a back pack or a pocket its a 50/50 shot.The blade is not exposed and cannot be exposed so as to be a threat but common sense and the TSA can't occupy the same space according to the laws of physics(grin).TSA web site should have a list of acceptable items on their website,You might also try calling the ResQme customer service and ask them they can probably tell you the answer too.

Would this be easy for a toddler to pull off the keychain and cut themselves with?

No the blade is in side a protective sleve has to be slid onto thin belt or paper to cut punch is covered with retractable cap and not exposed has to be pushed against window with about 3 lb or pressure to break glass it's save showed all my girls how to use on a wall my youngest is 10 it can be held in hand and blade will not cut you
